Question about output accuracy integrating circuit

I am trying to design an op-amp that is going to be used in an integrating circuit for integrating ADC. The trouble that I am having is that the integrating circuit's output is not accurate enough. For example, when the output is suppose to be 0.5V, it is 0.483V instead. Even if I use an ideal op-amp, the output is still not accurate enough. It is 0.494V instead of 0.5V. I would like the actual output to be at least 200uV within the theoretical output. Any idea how to do it?

what is your DC gain, bandwidth and UGB? It sounds like related to DC gain.
