The board I worked on had press fit on upper and lower edge so the template was for the entire board. I think as long as the SMT components near the press fit is not too tall them the recommended clearance is probably fine, but it all depends on how the template is made too.

I did couple of them while back also. The fixture was provided by the customer so I'm not sure what the cost is, but it was fairly simple metal plate with holes for the connectors so you can use a press to just push the connector together.

I believe Altium had issue with DXF import directly to where you would make the footprint. I believe I called it into the board data then copied it over to the footprint. I haven't used Altium for a while so if it doesn't make sense please ignore it.
Can you not just use a plated hole with pad and place the center on the board edge? I'm not sure, but I don't think you'll be able to make a half plated hole.

I would think contacting Cadence support would be the fastest solution. If you were able to do it before, then maybe reinstall? I suggest the support they are pretty responsive.
