It should be a NMOS transistor, but why is there a small rectangular of the symbol? What is the meaning of it? what is the difference between it and the normal NMOS transistor? Thank you!

I think somebody made it up.

The small rectangular of the symbol just shows the nonsymmetry characteritic of the HV NMOS.

I think it's the symbol of a lightly doped drain (LDD) HV transistor (feasible both with NMOS & PMOS). The small rectangle depicts the drain.
