Circulator Modelling in Cadence Virtuoso

Dear all,

I need a circulator (simple 3 port device) with the s parameters provided at figure below.

I was not able to find circulator model in libraries. I am planning to model it using nport or n2port from analogLibrary. How do I define the s-parameters (the s matrix same as figure). I don't have the s-parameters in a file format.
