If you need generic off the shelf amps got to:

Motorola,Freescale,Mini Circuits, Motorola,NXP, Skyworks, RFMD, Mitsubishi, Fujitsu, NEC, Mimix,...

If you need a "build to spec" type try Microwave Solutions, there are other
Amp Houses like MWS. I just don't know their names. Google is a great tool.
