PIC18F4550 Where To Begin?

Hi, I tried to make my own DIY circuit to upload bootloader into PIC18F4550. I follow the circuit from CUI "http://www.create.ucsb.edu/~dano/CUI/".

According to that website, first I need to connect the USB circuit to ICSP to run ICD2. Basically, I want to know how this "ICSP" looks like, can any expert out there show me how to make this so called "ICSP - In Circuit Serial Programmer"? Provides a schematic diagram is highly apreciated.

Why cant PIC18F4550 be programmed entirely using USB since it is USB full speed capable? Thx in advance guys...
