Did somebody worked on creating coverage plan for USB verification?

Yes, Functional coverage.

Added after 2 minutes:

Can you tell me what kind of coverage scenarios are possible for hub ?
i could see all the hub specific standard requests.
What else could be there?

I think you should study the features of your DUT (might be a HUB), then write the coverage points and coverage groups according to the features.

As you are planning for functional coverage and I understood from your post that you have DUT with hub features.

If I am right, you can step ahead as following.

1. First deside the group.
As your DUT is hub, "hub" could be your group
If your DUT is Host Cntrl, "inep" - IN endpoint could be your group as to check functionally IN endpoint

2. list all your features as per group and make the list of testcases
If your DUT is Host Cntrl, "inep" -IN endpoint could be your group and the possible testcases could be "bulk transaction", "control transaction", "interrupt transaction" etc.

So, finally one testbench from the possible testbenches considering DUT as host could be usb_inep_bulk_tb.v

Like these you can make the testplan for functional coverage....

I hope the answer gave you satisfaction.
