voltage regulator not working

thanks a lot to you all...
the things are working great... i have changed the complete circuit diagram and tried again with a new PCB... it works great..
Tried adding both input and output capacitor as close as to the IC.
the regulator is also not getting any hot and getting an output voltage of 4.99V.
