when changes in a design is new version and when it is new revision?

if you are doing minor revisions the task can be regarded as revision. if the design would require more time to revise or there are major changes it will be under revision.

Here’s my take on this:

Version number reflects the specification. If the specification changes, the version is incremented.

Revision number reflects the implementation. If the implementation changes without a spec change (e.g. a bug was fixed), the revision is incremented.
