Instead of searching or writing code for you...
a) use google and search for a similar project and modify it.
b) pay someone to develop the code for you.
c) develop the code yourself and use it as a learning opportunity to improve your design skills.
d) come back and ask specific questions when you get stuck while working on c or stuck with the second half of a.
BTW, c can be as simple as a bunch of block RAMs configured as a ROM and a counter to read the memory and output it to the DAC (with whatever interfacing and initialization requirements the DAC has)
