Question on offset cancellation in T/H circuit of ADC

Hi All,
Attached is a circuit I want to use in my first trying on ADC design. But a have a few questions:
1) Is offset cancellation needed for the circuit? If yes, how to add offset cancellation to it?
2) For the differential input, do I need some circuit to convert one end signal to differential one assume its amplitude is small, then the difference between VIN+ and VIN- should be orginal signal or 2 time the orignal signal?
3) How to connect in the common mode input voltage for the OTA?
