is this opamp available from any manufactures?

Hi, I need a low noise opamp with high bandwidth, the specification is very close to the following description. Does anybody know where I am
able to find it. Thanks a lot.

GBW: >100Mhz
Slew Rate: >200V/us
Vos: <300uV
Ib:<5uA
V noise Density: < 2nV/sqrt(Hz)
Vcc-Vee: >44 V
temperature range: -55 ~ 125
 

Go to the website of your favorite vendor, most of them have a part selector, type in your parameter. You will receive a list of parts which meet your specifications

but I think you will have hard time in general
perhaps somebody can design it for you - depends on volume.....
