how to avoid positive feedback in sound systems ?

how to avoid positive feedback in sound systems ? is there any circuitry for that or just organize the speaker system to avoid it
 

Frequency shift by +\-10Hz allows about 3dB increase in volume without feedback.

Automatic notch filter allows 6-10dB volume increase before feedback.

DSP echo cancelation allows maybe 30dB increase in volume before feedback.
