How is MCLR ground calculated by the 877A?

Got 2 ICD2's here both working apart from one has MCLR Gnd High on self check, I've been looking at voltages on pins and all sorts, can't seem to find a problem.

How is MCLR Gnd calculated by the 877A? does it have its own pin A4 for example, or is it the difference between two levels on a single pin? Wood for the trees problem this end I think.

Any comments welcomed.
