functional simulation does not work

i have problem with functional simulation in quartusII. it seems to be simulating but generated a lot of warning. most the output signals does'nt produced the desired waveform (i'm using the input stimulus via waveform, .vwi if i'm not mistaken). fyi, the VHDL code that i've written work on the other simulator, modelsim. and all the timing requirement is fulfilled.

so do i need to revise the code? or need to change quartusII settings, or else?

also, my reported simulation coverage is just 6.00%.
 

what simulator do you use?
 

Make sure that you have unckecked "Enable SignalTap logic analyzer".

first i used quartus-based simulation it failed. then i used modelsim, also failed.

This is a setting option in your project. (Menu Assignment/Setting...)
