Cadence- ams h35b4 kit-designing transimpedance amplifier for a photodetector

Hello everyone,

I am designing a Capacitive transimpedance amplifier for a GaN photodiode whose breakdown voltage is as high as 60-70V. I need to do a CMOS ROIC for the same. However, when I was designing, I needed to use a mosfet model that could withstand such a high voltage. I looked at nmosi models and thought I could use them to isolate the Low volatge CMOS circuitry from the input high voltage. But I have not used them before and I don't know what the fourth terminal (it has a diode kind of pin) should go to. Can someone who know explain to me how I could use this high voltage isolated model?

I am using the ams h35b4 design kit on Virtuose 6.1.5.
