4066 switch

I am looking for a similar quad switch like the 74HC4066 to switch between voltages of 0V, 5V, and 12V, and possibly 15V.

The 74HC4066 only allows up to 12V. Is there an alternative that I can use?

analog devices adg201 or adg202 are rated to +-15v
and even adg333 is rated to 44v
may be this is what you need !

also maxim max333 semms the same as adg333...

The CD4066 (74HC4066) is rated for voltages up to 18V dc, the recommended maximum Vdd voltage is 15V dc, and the input range is from -0.5V to 0.5+Vdd ..

If supplied of 15Vdc it should comfortably handle input/output signals of these levels ..
