A question about probability and reliability

I am trying to compute the probability of failure of the following system. "p" is the probability of failure. Note that system has two inputs and two outputs and some elements are shared between inputs.

Look at these two:

Failure rate - Wikipedia, the free encyclopedia
Probability - Wikipedia, the free encyclopedia

Just sum the ORs and multiply the ANDs

p(A and B) = p(A)*p(B)
p(A or B) = p(A) + p(B)
