I have a sine wave input to S1, but detect the point S2, the waveform turns to kind of square wave. Why is that? Thanks

You probably have too much gain in the second stage and signal is limiting there.
 

I think that the input signal level is too high for the amount of gain of the input opamp or the center opamp.

You can't see if the input opamp is clipping, or the center opamp is clipping or if both are clipping.
 

the signal is getting clipped due to the supply rails i think... if the supply range can be increased then u can increase it to get better output else you would have to reduce gain.....
