IC Measurement issues

Hi,
I developed a Class D power amplifier and it was made into an IC.I am trying to measure my IC.The output waveform should be a differential switching square wave whereas I am getting some weird waveform.Not only weird, those two weird waveforms from each output(Out+ and Out-) are not differential.

My simulated waveforms before the fabrication of IC were all perfect as how it should be.

I also added AC coupling stage(off chip) before my input stage in the measurement process.Since I didnt have any previous experience in measuring the IC before, I ask you people sincerely to give me some valuable suggestions and explanations as what I might be doing wrong.

I have a LC filter(off chip) after the output in order to carry measurements with an Audio Analyser.
