How to use the three terminal resister in the model lib?

The resistors in the library consist of two types; diffused and insulated. The
diffused types include n+ and are 3 terminal resistors with diode backplates.

Two terminal of the resistor link to input and output port?

Did No.3 terminal link to vdd or gnd?
diffused and insulated typy are the same?

Most of the resistor can contain 3-ports like in High K poly resistor or n+/p+ resistors. You are right. The third port tends to connect to either VDD or VSS depending on your design. Say, if you want to reduce substrate noise effect (assume you have common substrate), then connecting to VDD is a better choice.
However, some ppl will have fixed bias say from regulator of VCM to connect to have some stable bias as well. So, it depends...
