Output stage
Yes.. the low pass filter must be at least 20 or 30khz to allow audio signal.
The output stage need to be very very large,.. you need buffer for that.
Errr.. actually I am not sure whether this is a good idea. But from the waveform obtained, it seems there was high frequency component in the waveform. So I added small caps just to remove those unwanted 'distortion'.
What do you think?

I am designing this class D amplifier with feedback. The THD is pretty high.
I am really wondering the causes of the THD. I know it should have come from the non-linearity in the system. I hope to reduce it to minimal, but really got problem.
Do anyone design it before? Or any...
it will lower the noise if it is inside the system, eg. power supply noise or distortion within. Further analysis is described in the Sergio Franco textbook. I read it there.
Re: MOS as switch question
The NMOS must be fully turn ON. When it is working as a switch, it will act in triode with low resistance path.
You can check the digital ic notes.

I am designing this PWM using comparator and triangular carrier.
I am a bit confused about the duty cycle. I roughly know it is the ratio of high and low over 1 period. But, how do I know the overall duty cycle? What is the meaning of zero-input duty cycle?

Thank you for the input, but this is actually done in a simulation.
The op-amp is self-designed, it is a folded-cascode, single stage amplifier.
That is why I thought it is the op-amp-problem.
The pulses is not clean waveform, there are some jitters at both low and high voltage.
