Using EEPROM for waveform generation

I want to use the EEPROM to store certain binary values and generate a typical waveform.
Can anybody tell me how to do this.
How to write the code in the hex editor.
I am using Proteus for simulation.

I want to store this pattern in EEPROM and access it i.e. when address on EEPROM pins is 000000 it should output 0 when address is 000001 it should output 1 and so on upto 111011.
