If you like to do it yourself : http://www.compsys1.com/workbench/On_top_of_the_Bench/MMC_Project/mmc_project.html
You can order too.

If you like ready made:

Based on AVR Mega128 for $95 and FlashFile SD/MMC FAT12/16 software in C for additional $139 : http://www.prllc.com/productcart/pc/viewPrd.asp?idcategory=5&idproduct=9
A little bit expensive the software but if you like AVR microcontroller, you can ask nice and someone on forum will help you.

Based on Atmel AT91SAM7S64 (picture bellow) for $60 : http://www.olimex.com/dev/sam7-p64.html
Schematics and software included on web site.

If you like Philips ARM then : http://www.olimex.com/dev/lpc-p2148.html for $75
