[SOLVED] problems in designing ota based notch filter

i want to design an ota based notch filter that is going to remove the 50 hz noise i have designed something but its not working can you please point out the mistake it would be very helpful the gm of my ota is 314.6u

Did you determine OTA common mode range? As far as I see, input source DC offset 0 is outside common mode range.

Center frequency seems to be off by factor of 10.

icmr max is 1.6v and Icmr min is 0.8v can you please tell me what i need to do
Add DC offset of 1.2V to V3. Check X1 and X2 Vout.
