Well, I've heard sg @ GemPlus. They produce smartcards for various purposes (GSM-SIM, SAT receiver etc.) After packaging the chips (to the plastic card) an "OS" will be uploaded to the card. This is a specialized application for the target (f.ex.: SIM) This will be the SAME for ALL cards in a shipment.
During the personalization process they will upload at least the IMSI and the Ki (in case of SIM).
