Patient is falling down/has fallen down using RFIDs

Hi,

I want to design a system which tracks mental patients.

1. Their location
This can be tracked by placing multiple readers and tag responds to it. By checking the maximum signal strength, it can be stated that a patient is located.
Secondly, it has to trigger an alarm when patient is moving towards restricted areas. The above mechanism can be used to serve this.

What is the mechanism to be sketched in order to know when a patient is falling down/who has fallen down?
We can check whether the patient is immobile for a longer time. But it doesn't serve the requested scenario. Even if the patient is sleeping, it will trigger an alarm in that case. What mechanism can be devised?
