Offline 198W flyback transformer with just 0.5% leakage.....not possible?

Page 40 of the following

....shows a transformer for a 198W flyback with Vin = 90-270VAC.

This transformer has a leakage inductance of just 0.5% of primary inductance.

Surely, this is not possible with simple single sandwich winding as they have done there?

With care you can achieve even better results, but not by much (of course, other's experience may vary). That simple sandwich is just a picture to you, but the way it's done means the difference between a burned-out transistor and an excellent supply. If I may, I have a hunch you're considering first some "rules of thumb"; ditch them, they are only good to "get a feeling" of what something is about, they're not good at all when it comes to details. Saying "that doesn't seem right" doesn't mean it actually isn't until a closer look reveals the truth.

A practical example that comes to mind is bread: you only need water, salt, yeast and flour, surely nothing good can be done with only these, no?
