MAX4315 - help needed

Anyone here who has an idea or reference schematic of MAX4315 (High-Speed, Low-Power, Single-Supply Multichannel, Video Multiplexer-Amplifiers).

I need this for DVR-SPOTOUT applications.

You can refer to attached datasheet.
