How does Altium/Mentor graphics tool automatically generate test points

what approach the tool follows internally to generate a test point on the layout?
 

In Altium having test point feature, go through PCb--> design--> rules--> test point, here set the test points of fabrication and assembly test points with no grid enble all RULES Scope HELPeroptions, and go back to tools--> test point manager here click on Fabrication Test points and Assembly Test points, and try to generate test poins on layout.
