help me on the problem of the simulate of a simple adc

this is a 4-order cascading adc, a simple 4-order adc
with matlab i find a problem .
first
the input of the second stage is too large (see the pic)
second
the last output is in the arrang of -1 and +1,but it should be in 0-1
why ?
what wrong?
