DC Motor powering rotary actuator with diodes - diode fails

This is the wiring diagram to a rotary actuator I am experimenting with.

The red and black wire power the actuator, to get it to position A, you have + to red, - to black. To get it to position B, you reverse polarity.

Wires blue and white are for feedback.

At room temperature, I had a diode failure, it looks like it overheated and expanded from the inside until it became an open circuit.

I was hoping you folks could offer me some insight into what the problem could be.
