couldn't anyone help me with this shit of microchip?!!!

microchip icd0161 verify failed

could you please help me i have problem
with dspic30f6014A programming.
after first time programming it , then ICD2 can't identify
the chip and also it couldn't be erased or reprogrammed.
the icd2 error is when cheking blank:
MPLAB ICD 2 Ready
Programming Target...
...Validating configuration fields
...Erasing Part
...Programming Programming Executive
...Verifying Programming Executive
ICD0161: Verify failed (MemType = Test, Address = 0x800000, Expected Val = 0x40080, Val Read = 0xFFFFFF)
