There have to be 100's of books on the subject and numerous software packages as well.

It is Phillip H. Smith. Electronic Applications of the Smith Chart. It was first publish by McGraw-Hill in 1969. Then Noble Publishing republish it in 1995 and 2000. I believe this book is widely available.
