How to improve OP400A CMRR

In OP400A device how to improve CMRR by using 3 stage servo circuit.
input resistor are used 0.1% tollerence,and common mode volatge is +/-12V but results are getting 120dB max how to improve for this device.
circuit is aatched as below.

There's no common mode voltage source in the circuit. Please clarify.
 

V7 source is the common mode voltage provided as 12V
 

Makes no sense. OP400 common mode is only set by supply voltage sources V1 and V2.
