Extraction does not include the drain/source cap. However, some old extraction tool DOUBLE count the drain/source area when you share them in the layout but use hierarchy netlist. Later HSPICE will see bigger diffusion capactance. If this is the case, either update your tool or flatten your netlist.
Re: Slew rate control
Usually I/O circuits has slew rate requirement. Check those reference.

Re: Bandgap design
Si bandgap voltage has three term, one constant, one linear and one ln.
Zero Order: no temperature compensation is implemeted and thus is the simplest bias circuit such as a forward-biased diode or zener reference.
First Order:cancel the linear term by combining the PTAT...
failed to initialize tcl interpreter hsim
I got the same message but hsim itself works fine. Does this bothers you? But I'm glad if someone can fix this issue. Anyway, hsim doesn't use gui, so what's this tcl for?
For a cascode, the gain is approximately
= gm*ro^2 = gm * (1/(Lemda*Id))^ So if you keep the bias Id unchanged while increase the width you can boost your gain. However, large transistor increase the node capacitance hence limit the bandwidth. Run the simulation to dertermin the trade off.

gain bandwidth product 741
741 is probably the first opamp ic popularized. Its 3-stage design reprensents the essencial of modern opamp topology. Thus it serves an excellent turorial to beginners.
