In spectre each voltage source typiccaly has parameter "DC voltage" this voltage will be used by DC simulation. Set this voltage to what it needs to be.

To me it looks like amp is falling over on corners. When you run corners do you see similar phase behaviour?
Can you run monte carlo mismatch only?
Extractions can sometimes be missleading. 4p is way too large capacitance. You can check it by extracting and by old good hand calculations with the ruler and some number from the process.

You need to have offset in your amp so that you can see larger pop/click. Do you simulate with an offset?
