for amplifying , use the NON INVERTING AMP, Select the gain to 50, to achieve this mark. But it must be taken care that , if the 2mV signal is induced with noises, then the same will also get amplified, so better to use a filter along with that. With the same Opamp, you can a make an active filter.

I just tried by reducing the PWM frequency. Now its seems to the earlier 30% duty cycle margin is increased to 50% without heating. I believe its because of the Bootstrap capacitor's charging & discharging timings.

Check the datasheet for the DTMF decoder IC MT8870. By interfacing the output pins of this IC to a microcontroller, you decode the DTMF tones and do the necessary job for your IVRS requirement.
