TI064 troubles

Hi, I am trying to use a simple non-inverting opamp configuration to bump a 5V pulse at 10Hz to 10-20V. I hooked up the output of a digital pin of an arduino to at TI064, and used +/-15V for the supply rails. I am consistently getting a larger output (typically ~5V) more than expected (tried adjusting the resistor values a few times). What am I missing?

We can see the input pulse is 0V / 5V.

Now please write clearly what output levels you expect.

* rather interesting "double inverted" positive power supply connection..
* missing R2

Notice the limitations of the output swing, above from spec sheet.

What is the G of the opamp circuit ? See following :

So what would the output want to be but cannot because your G is too high and is driving the
output into the rails. So the output transistor, Q14, in this case, saturates and cannot provide anymore
voltage to its output and or a load connected to output. The below OpAmp internal circuits are
"typical" for this example.
