temocouple and PT100 to 4-20mA conversion

Please give me the IC part numbers that can be used for termocouple(J,K,T,R,S) ,RTD(PT100), to 4-20mA transmitter.

Is there any single chip or 2 chip solution.
Please help me for a good solution.

Or instead of expensive current transmitters from TI or ADI, some discrete designs with cheaper parts from National or Maxim.
