Simulation error in cadence(spectra)

Simulation error

Hi. recently i doing some simulation on the analog design using cadence(spectra). after finished running the simulation spectra give the error on the schematic beside the transistor.

what is the error mean and how to fix it? can anyone tell me? i include the pic of the error. thank you.

in this u have shorted the substrate and the source over the symbol of the transistor. i have editted ur picture... try connectiong it like this
