Separate ground for analog and digital signals

what is the need for separate groung for analog and digital signals?
(in ADC chips v have so)

To make sure the switching noise created in digital circuit will not disturb the analog signal...

for the analog circuit not to be disturbed by the high frequency signal in the digital part.
but how does it work? Before the digital signal is linked to ground, are there some processing procedures?

We also use different pins and different vias for analog layout and also our Vdd vias and pads are different from digital one's ....
