Missing Injected Instance Error in Calibre LVS

I've encountered a "missing injected instance" error in after caliber LVS. I've checked all the instance manually. The instances in schematic and layout are matched. But I'm not able to figure out, what it's really pointing out. Can anyone help me figure out this. The snippet is attached.
