Lmh6551 fully differential amplifier

hi,
i use the LMH6551 like the schematic attached.

with power 12V the LMH6551 burns and Vcm goes at 11V.

i do not understand whi this appens,because in the datasheet the maximun supply voltage is 13.2V

anyone as any idea?

What voltage have you driven to Vcm? It cannot be left floating, I think.
