ideal balun in layout

Hello everyone
i have generated layout of OTA design shown below. DRC is sucessfully run without ideal_balun
But i dont know how to connect ideal_balun in layout. please help me i got xfmr component error in LVS.
I have removed the ideal balun and put voltage sources with pins connection.
but problem is that i want to apply both differential voltage and input common mode voltage simultaneously. how can i apply it?
How can i solve it? thanks in advance

Seems you didn't study the paper I suggested to you? P. 8 exactly shows the test bench for this purpose: "Simplest Test Bench is Unity Gain Circuit".
