COSEL make power supply model LDC30F1 not working

COSEL make power supply model LDC30F1 not working due to missing of gate pulse of 5v for operating pulse for power MOSFET K2045. I also replaced K2045 & optocoupler NEC2561 but problem still prevailing.

Kindly do me a favour to rein in the problem. Moreover provide the schematic of the same.
