The HEX files that are included with revB and revC have MCLR enabled on 18F4550, but neither PCB has a resistor from MCLR of 18F4550 to Vcc. The attached HEX has MCLR disabled, therefore the resistor is not required anymore.

Schematic diagram for ICD2 clone. rev C. Note missing R from MCLR to Vcc. You can either use a R, 10K, to Vcc or use my HEX (look above) to disable MCLR. I believe the whole package, with Gerbers is posted somewhere in edaboard.
