about adc gain error

how to understand the gain error of adc? i think, it is the change of slope of actual transition curve relative to ideal transition curve. but corresponding to the output code, what does it mean?
for example. for a 12 bit adc, the ideal max output code is 4095, but if the actual max output code is 3957, while the min output code is 138, and the gain error is +/-5%, is this feasible?
pls explain to me, or give me some related advice or suggestions, thanks all.
