How we can decode a damaged code ?

Decoding(Help)

Hi
Is there any one who know how we can decode a damaged code which may have lost bits or unwanted extra bits. this situation could bae occoured because of bad hard decision of noisy signal which loss bit(s) or produce extra bit.
please help me.
