In the US at least, you can replace the center screw that holds the cover on your outlet with a longer one, then run a wire from the mat to the center screw with a ring tongue terminal. If the outlet is wired correctly, the center screw should be grounded.

It may be easier to just replace parts than to try to diagnose. Remove and replace all the electrolytics and look for damage under them from leaking. Maybe replace all the Zener's, they're cheap. Replace the LM350 if you can, it's the only active part.
I've never played with them, but isn't there a limit to how much of a temperature change you can have across one plate? Maybe try stacking another plate with the first one to boost your cooling.

If you're driving a motor (as FvM said, an inductive load) the relay will be severely de-rated from the maximum resistive load that's usually printed on the relay. Make sure you look up the maximum motor size you can switch.
